A useful partnership starts with defined responsibility, a recognizable patient experience, and visible follow-through.
The partner’s clinicians should retain assessment and treatment decisions. Navigation supports the care plan and routes concerns to the appropriate clinical team.
For an appointment, refill, referral, or clinical instruction, identify who follows through and where the outcome is documented. A message sent is not always a task completed.
Agree on the introduction, calling identity, and explanation of the service. Patients should understand who is contacting them and how the team relates to their clinic.
MySeema’s default is to work inside the partner’s existing EMR. Access, documentation, escalation, and reporting are defined before launch.
MySeema’s monthly reports include care touches per patient, care plans completed and reviewed, follow-up actions taken, and patients routed back to the clinic.
