Which patients fit MTM?
  • The patient is enrolled in a Medicare drug plan and is eligible for that plan's MTM program under its current criteria.
  • The patient needs help connecting with the plan or preparing for a medication review.
  • Medication questions, a medication list, or action items need organized follow-through with pharmacists, prescribers, the plan, or the care team.
What must be in place before launch?
  • Current Part D plan identification and MTM eligibility confirmation
  • Clear route to the plan's contracted pharmacist or other qualified MTM provider
  • Patient permission and secure medication-information workflow
  • Named route for recommendations and medication concerns to reach prescribers
  • Explicit separation between MySeema coordination and the plan's qualified MTM service

Coordination support only · Part D plan and qualified-provider delivery must be confirmed

Billing details, patient costs and service boundaries
  • No national PFS rate is supplied. Part D sponsors arrange provider payment by contract.
  • Eligible beneficiaries receive plan MTM without an additional patient charge.
  • Any MySeema coordination fee is set by written agreement. This page has no MTM estimator.
