IntraCare, an accountable care organization (ACO), chose MySeema for care management and navigation across its Medicare and value-based care population. The partnership brings remote monitoring and ongoing coordination into one operating relationship.

The public record
The announcement says MySeema would assume operational responsibility for IntraCare’s care management suite, including its live remote patient monitoring program and the build-out of CCM, TCM, APCM, and additional services.
MySeema and IntraCare’s clinical and operational leaders would design navigation and engagement pathways for attributed patients, including people who do not qualify for a specific reimbursable program.
The announced initiative supports IntraCare’s Medicare Advantage and Medicare Shared Savings Program populations through remote monitoring, proactive coordination, and longitudinal care management.
The announcement defines partnership scope and intent. It does not report enrollment totals, measured clinical improvements, utilization changes, or savings produced by the partnership.
The service model
In MySeema’s current partnership model, navigators handle assigned outreach, practical coordination, and documentation within participating care teams’ EMRs. A U.S.-based RN reviews concerns under agreed clinical protocols and routes them to the appropriate clinician when needed. This is managed operating capacity, not simply access to software.
The treating clinicians retain assessment, treatment decisions, and clinical oversight. Before patient outreach begins, the partner and MySeema agree the population, responsibilities, introductions, record access, documentation, and escalation pathway.
For ACO leaders
An ACO can discuss a defined patient population or workflow, the care management programs and navigation support that fit, how work should be documented, and what operating measures the team will review. The proposed scope, responsibilities, fees, and evaluation approach are agreed for each partnership.
