Who these terms cover
These terms describe direct individual and family services offered by MySeema. They cover monthly memberships, single remotely supported doctor appointments, and gifted visits. They do not govern provider-sponsored Medicare programs or replace a signed organizational agreement.
An adult may arrange support for themselves or another person who agrees or for whom they have legal authority. MySeema confirms the service and applicable terms before enrollment or payment. The person receiving support controls authorized updates, subject to lawful representative authority. A payer or gift buyer is not automatically entitled to care information or medical decision-making authority.
Existing members retain the terms already accepted unless a change is lawfully made with the required notice or consent. These website terms do not retroactively remove an existing contractual benefit.
Coordination, not diagnosis or treatment
MySeema provides care navigation, coordination, and care-management support. We help organize appointments, communications, records, and follow-through. Our navigation service does not diagnose conditions, prescribe medicines, or decide or change medical treatment. Those decisions belong to your treating physicians and other appropriately licensed treating clinicians.
Remote attendance means a MySeema team member joins your appointment from another location to help prepare or ask questions, take notes, provide a summary and next steps, and coordinate the agreed follow-up. It is not a medical consultation provided by MySeema. Your treating clinician remains responsible for medical advice, diagnosis, prescribing, and treatment.
We do not replace hands-on caregiving, transportation, an emergency contact, or a legal healthcare representative. Summaries support communication and are not guaranteed to contain every detail of the clinician’s record. Confirm unclear medical instructions with the treating clinician.
Monthly memberships and included support
Routine · $79.99/month
Scheduling and prescription-refill requests by text, as requested. No remote doctor-appointment attendance, phone calls, appointment advocacy, provider research, or visit summaries are included in the monthly fee. Remote appointment support can be purchased at the member rate.
Proactive · $149.99/month
Remote support at 6 doctor appointments per 12 months from membership start, with preparation, questions, summaries, records, and coordination tied to those visits. Includes text support, up to one brief monthly check-in call, and provider research when a new provider is needed.
Companion · $199.99/month
Remote support at 10 doctor appointments per 12 months from membership start, plus ongoing coordination, advocacy, provider research, records, text and phone support, FaceTime, and proactive check-ins across the care team. Unlimited provider-visit summaries and coordination are separate from the 10-attendance allowance. After-hours limits apply.
The 6- and 10-appointment allowances run over 12 months from your membership start date, not per month or calendar year. The monthly fee pays for the plan’s described support while membership is active. It does not purchase unlimited remote attendance. Your enrollment confirmation addresses the treatment of unused allowances if you change plans, cancel, or restart; ask about this before enrollment.
Companion’s unlimited coordination and provider-visit summaries describe the types of support included. They do not mean continuous availability, guaranteed completion by a particular time, unlimited remote attendance, or medical treatment. Scope and normal after-hours limits still apply.
The Families page explains the plans in more detail. A request beyond the agreed scope requires discussion and agreement on any additional charge before it is provided.
Single visits, gifts, and add-ons
One remotely supported doctor appointment costs $49.99 for a non-member buyer or $29.99 for a current member buyer. The member rate also applies to extra visits after a Proactive or Companion allowance is used and to Routine members purchasing a visit.
For gifts, the buyer’s membership determines the rate, even when the recipient is not a member. A gifted visit requires the recipient’s agreement and appropriate permissions. MySeema confirms availability and appointment arrangements before payment. A single or gifted visit does not automatically enroll the recipient or buyer in monthly membership.
Enrollment, billing, and renewal
Membership fees are paid directly and billed monthly. Once you affirmatively enroll in recurring membership, it renews each month until cancelled under the cancellation terms below, subject to applicable law. Before you authorize recurring charges, we provide the plan, price, billing interval, start date, cancellation method, and applicable terms for your express agreement. An inquiry is not that agreement.
You are responsible for authorized fees and any separately disclosed applicable taxes or charges. Costs charged by doctors, pharmacies, laboratories, insurers, transportation providers, or other third parties are separate from MySeema’s fee. Membership is not insurance and does not guarantee coverage, reimbursement, or payment by Medicare or another insurer.
Material changes to accepted prices or renewal terms require the notice and consent applicable law requires. We provide required renewal reminders and cancellation information. Do not send full payment-card numbers or security codes through the website inquiry or ordinary email.
Cancellation and refunds
30 days’ notice is required, except where applicable law gives you an earlier cancellation right.
Give at least 30 days’ notice by emailing support@myseema.com or sending a clear cancellation request through your established MySeema text conversation. Unless you request a later date, the effective cancellation date is 30 calendar days after we receive your notice, subject to any earlier date required by law. We confirm your last billing date and the end of your paid service period. The notice period is not extended while we process your request. Scheduled monthly charges within the notice period remain payable unless applicable law requires otherwise; no new renewal charge is due on or after the effective cancellation date. Access continues through the period already paid for. Where applicable law requires earlier termination or stopping renewal immediately, we honor that right instead of the 30-day notice requirement.
Payments are non-refundable except where required by applicable law. There is no free trial or money-back guarantee for new purchases under this offer. This policy does not remove a right to dispute an unauthorized or incorrect charge, receive a legally required remedy for services not supplied, or claim a benefit expressly promised in an earlier accepted agreement.
Email a cancellation request →
You do not need to provide a medical reason, accept a retention offer, or attend a call to request cancellation. If you use a different email address from your account, reasonable verification may be needed to prevent unauthorized changes. A clear cancellation request can also be made through your established MySeema text conversation. Keep your sent message for your records.
Appointments and third-party cooperation
Remote participation requires appropriate consent, the provider’s cooperation, and a workable connection. Tell the team about the appointment and any office requirements in advance. MySeema cannot override a provider’s rules or guarantee that a third party will accept remote attendance.
If an office declines participation, an appointment changes, or a connection fails, contact the team so we can discuss available alternatives, rescheduling, and any remedy required by the accepted arrangement or law. Do not delay necessary medical care while arranging MySeema participation. No additional service or fee is authorized merely because an appointment changes.
Provide accurate contact and appointment information, relevant records or permissions needed for the task, and changes that affect the support. Do not share portal passwords with a navigator through an ordinary message; arrange appropriate authorized access. MySeema is not your treating clinician’s emergency or results-notification system.
Permissions, notes, and communication limits
We arrange communication channels and authorized contacts with the person receiving support or their lawful representative. Permission to join a visit or send family updates can be changed prospectively. Recording or automated transcription requires any legally necessary notice and consent before it occurs. Remote attendance does not by itself authorize audio or video recording.
Texts, calls, and email are for the agreed navigation support. Carrier message and data charges may apply. You may ask to change communication preferences or stop nonessential messages. If you also want to end membership, make a clear cancellation request.
Messages outside normal business hours, including weekends and holidays, may not be reviewed until the next business day. There is no promise of an immediate response or continuous monitoring. For an emergency, call 911 or go to the nearest emergency room. For urgent medical questions, contact your treating clinician or their after-hours service.
Service limits and rights
Navigation depends in part on information and cooperation from other people and organizations. MySeema does not guarantee health improvement, appointment availability, insurance decision, or financial saving. We remain responsible for the support we agree to provide and for duties imposed by applicable law.
Service may need to be paused or ended for non-payment, unsafe or abusive conduct, lack of required permission, or a need outside our scope. We communicate applicable next steps and handle records, paid services, and any required transition or remedy under the accepted agreement and law.
Nothing here excludes a non-waivable consumer, privacy, or professional right or excuses fraud, gross negligence, or willful misconduct. These terms do not impose arbitration or a class-action waiver. Texas law applies to the extent permitted, while mandatory protections where you live continue to apply.
